Preliminary Diagnosis Form for Hotels

Terms:

• The present Preliminary Diagnosis Form, can not be published at any given time, entirely or in parts, and/or under any circumstances in any language in digital and/or in print by the hotel management.
• All information supplied by the hotel management in the present Preliminary Diagnosis Form will be considered valid and true at the date the form is submitted.
• All information supplied by the hotel management can not be published by NaturCert without the written approval of the hotel management.
• The hotel management reserves the right to publish anywhere at any given time in any language in digital and/or in print the Preliminary Diagnosis Report, which the hotel management will receive by NaturCert in PDF format, by mentioning it is supplied by NaturCert.

I have taken full knowledge of all the above
